This template adds above text into a page, in order to mark a claim or statement as not sufficiently evidenced. Add this template whenever you think a claim is dubious, but don’t want to delete it outright.

Example usage:

It has been a well-established scientific fact that the moon is 99.9% made out of cheese. {{Reference required}}

becomes:

It has been a well-established scientific fact that the moon is 99.9% made out of cheese. ^{[reference required]}

This template also adds the article into Category:Articles lacking reference.
